Your invitation.
Your information.

This notice covers the Family Health Office beta invitation form, an OpenWorkLab project. Updated October 24, 2026.

What the form saves

We save your email address, your choice to receive beta emails, the signup date and the version of the consent notice. We use these to manage private beta invitations and contact you about this project. Joining the list doesn’t create an account, enroll you in medical care or confirm a place in the beta.

No health information at this step

Please don’t send medical records, symptoms, family details or passwords through this form. The examples on the landing page are fictional illustrations of the product we’re building. They aren’t live connections to health or calendar accounts.

How we handle it

The email is encrypted in the invitation database. Authorized project operators can access it to manage the beta. We use OpenAI Sites and Cloudflare to host the page and process submissions. Hosting services may process routine connection and security information, including network addresses. The form uses short-lived tokens and rate limits to reduce abuse. We don’t add advertising trackers or sell the signup list.

Your choice

You can withdraw your request or ask us to delete it by emailing brandon@openworklab.com from the address you used to sign up. Requests expire after 180 days. Expired records are removed during the form’s next cleanup. We’ll ask separately before collecting health information or connecting accounts during any beta.
